MODULE 5: Tolerability Considerations with ADCs for Breast Cancer

  • Spectrum, incidence and severity of common and unique adverse events (AEs) with different ADCs employed in the management of breast cancer
  • Monitoring and management of acute “chemotherapy-like” AEs reported with ADCs, such as cytopenias and gastrointestinal events
  • Recommended algorithms for mitigating more serious (eg, interstitial lung disease, left ventricular dysfunction) or unique (eg, oral mucositis/stomatitis, ocular AEs) toxicities documented with one or more ADCs
  • Strategies to distinguish the cause of AEs that could be attributable to either agent in ADC-containing combination regimens
  • Impact on tolerability, if any, of the placement of ADCs in the treatment course (eg, later-line therapy for metastatic disease, first-line therapy for metastatic disease or neoadjuvant/adjuvant therapy)

Target Audience
This activity is intended for medical and radiation oncologists, hematologists, hematology-oncology fellows, general and breast surgeons and other healthcare providers involved in the treatment of breast cancer.

Learning Objectives
At the conclusion of this activity, participants should be able to

  • Assess available Phase III data with HER2-directed antibody-drug conjugate (ADC) therapy as a component of neoadjuvant therapy for patients with high-risk localized breast cancer, and consider the current clinical role of this novel treatment approach.
  • Evaluate current research evidence with HER2-directed ADC therapy for patients with HER2-positive localized breast cancer and residual disease after neoadjuvant treatment.
  • Appraise available research data and relevant clinical and biological factors guiding the selection of first-line therapy for patients with newly diagnosed HER2-positive metastatic breast cancer (mBC).
  • Review published research supporting TROP2-directed ADC therapy in combination with anti-PD-1/PD-L1 antibodies for triple-negative mBC, and use this information to make appropriate treatment recommendations.
  • Evaluate published clinical research findings with TROP2-directed ADC monotherapy for HR-positive and triple-negative mBC, and optimally incorporate these agents into clinical care.
  • Assess the biological rationale for the evaluation of HER2-directed ADCs for HER2-low and HER2-ultralow mBC, and identify patients appropriate for this treatment approach.
  • Discern the side effects and toxicities associated with FDA-approved ADCs in the care of patients with breast cancer, and identify strategies to manage and mitigate these complications.
  • Recall ongoing trials evaluating the potential role of novel ADC-based strategies in the localized and metastatic settings, and appropriately counsel patients with breast cancer regarding enrollment.

LEARNING OBJECTIVES

  • Evaluate relevant biological, patient and treatment-related factors to personalize the selection and sequencing of therapy for individuals diagnosed with hormone receptor (HR)-positive, HER2-negative metastatic breast cancer (mBC) harboring PI3K/AKT/PTEN pathway abnormalities.
  • Appreciate the pathophysiology, frequency and severity of hyperglycemia associated with the administration of various agents targeting the PI3K/AKT/PTEN pathway in HR-positive, HER2-negative mBC.
  • Recall strategies commonly employed to mitigate and manage hyperglycemia in individuals receiving treatment with agents targeting the PI3K/AKT/PTEN pathway, and use this information to appropriately intervene when this side effect is suspected or diagnosed.
  • Appraise the role of multidisciplinary specialists such as endocrinologists in the diagnosis and management of hyperglycemia associated with agents targeting the PI3K/AKT/PTEN pathway, and effectively collaborate with these clinicians to offer best-practice care for patients at high risk for or with a suspected or confirmed diagnosis of this toxicity.

TARGET AUDIENCE
This program is intended for medical oncologists, breast surgeons, radiation oncologists and other healthcare professionals involved in the diagnosis and treatment of breast cancer.

LEARNING OBJECTIVES

  • Appreciate the key clinical variables affecting preoperative therapy and the selection of a specific neoadjuvant regimen for patients with HER2-overexpressing localized breast cancer, and integrate this information into treatment decision-making.
  • Assess available Phase III data with HER2-directed antibody-drug conjugates as a component of neoadjuvant and adjuvant therapy for patients with high-risk localized disease, and consider the potential clinical role of this novel approach.
  • Evaluate published research data to guide the selection and duration of adjuvant and/or extended-adjuvant therapy for patients with HER2-overexpressing localized breast cancer.
  • Appraise available research data and clinical and biological factors to guide the selection of first-line therapy for patients with newly diagnosed HER2-positive metastatic breast cancer (mBC).
  • Appreciate the key clinical variables affecting the selection and sequencing of therapy for patients with relapsed/refractory HER2-positive mBC, and use this information to personalize treatment recommendations.
  • Design an optimal approach to the care of patients with HER2-positive breast cancer and CNS metastases, evaluating the implications of prior treatment exposure, symptomatology, extent of disease and other factors.
  • Recognize common and rare side effects associated with approved anti-HER2 agents, and use this information to develop supportive management plans for patients undergoing treatment for HER2-positive breast cancer.
  • Assess ongoing clinical research studies evaluating novel agents and treatment strategies under development for HER2-positive breast cancer, and counsel patients regarding the potential benefits of trial participation.

TARGET AUDIENCE
This program is intended for medical oncologists, breast surgeons, radiation oncologists and other healthcare professionals involved in the diagnosis and treatment of breast cancer.

LEARNING OBJECTIVES

  • Evaluate recently presented clinical research findings to determine their effect on the current management of localized or metastatic breast cancer (mBC).
  • Appraise published efficacy and safety data from randomized clinical trials evaluating the use of CDK4/6 inhibitors for hormone receptor (HR)-positive localized or metastatic breast cancer in order to counsel patients appropriately regarding the optimal clinical use of these agents.
  • Recognize the frequency of PIK3CA/AKT1/PTEN alterations and ESR1 mutations in patients with HR-positive mBC, and employ evidence-based approaches designed to target these aberrations.
  • Understand the mechanism of action of, published research findings with and current and future clinical role of oral selective estrogen receptor degraders for patients with HR-positive mBC harboring ESR1 mutations.
  • Evaluate published research findings to effectively inform the selection and sequencing of available therapeutic agents and regimens for patients with HER2-positive localized and metastatic breast cancer.
  • Appreciate the incidence, characteristics and clinical relevance of HER2-low or HER2-ultralow mBC, and understand available research findings with HER2-directed antibody-drug conjugates for these patients.
  • Interrogate published Phase III research documenting the efficacy of TROP2-directed antibody-drug conjugates for patients with mBC to determine the current and potential clinical applicability of these approaches.
  • Assess the mechanisms of action of, early data with and ongoing clinical trials evaluating other novel agents and treatment strategies under development for localized and metastatic breast cancer.
